Wellspring Physical Therapy is proud to say you will receive individualized treatment sessions with someone who really listens to you. The information below will help you to prepare for your first appointment with us.

To Schedule An Appointment:

In Arkansas, there is direct access to physical therapy, which means you can come directly to a PT without having to see your physician first. At Wellspring Physical Therapy we work with you, your primary care physician, dentist or doctor. If you do not have a doctor, and it is indicated to see one, we can assist in getting you to see the appropriate doctor. Your insurance may require a prescription from your doctor before they will pay any benefits.
If you do not have a prescription and are unsure if physical therapy is appropriate for you, we offer a free phone consultation. Through this screen we assess your condition, determine your needs and begin a dialogue with your physician to obtain a prescription to begin physical therapy. If you have already scheduled an appointment with us you may print out the forms below so that you can save yourself time when you come for your first appointment. Please call our office so that we can verify your insurance. We accept many insurance plans. If you don't have insurance coverage you may be seen as part of our prompt self-pay program. Our prompt self-pay rates for physical therapy are $125/visit. You will be required to pay at the time of service. We accept cash and personal checks.

What to expect on your first appointment:

In a few days you will have your first physical therapy appointment. You should arrive 10 minutes early for your appointment and wear clothing that is loose fitting. Please fill out the information form, provided above, and bring it with you to your evaluation. Your therapist will be asking you many questions regarding your complaint(s). He/she will also take range of motion measurements, and perform special tests as it relates to your diagnosis. The therapist will gather as much information from you through the evaluation process to formulate an individualized therapy plan. Your point of view and goals for recovery are important. You will also be given exercises to work on at home which are imperative to your recovery.
Your physical therapist will continually reassess your progress toward your treatment goals and will work with you and other health professionals to plan for your discharge from physical therapy.
